Case History 

 A 40 yr OLD MALE PATIENT WAS BROUGHT TO THE CASUALITY IN UNCONSCIOUS STATE SINCE 10 AM ON 22/9/21 2:00pm
PAST HISTORY:
PATIENT WAS APPARANTLY ASYMPTOMATIC 2 DAYS BACK THEN HAD A HISTORY OF GIDDINESS, SWAYING, HEADACHE UNEASINESS NAUSEA HE VISITED THE HOSPITAL ON THE NEXT DAY WITH SIMILAR COMPLAINTS ON PRESENTATION (IN OTHER HOSPITAL), 
PATIENT WAS CONSCIOUS, BP :180/100 MMHG, 
SPO2 :98% 
 CT BRAIN: SHOWED LARGE HYPODENSE AREA IN THE CEREBELLAR HEMISPHERE SR CREAT: 0.9
GRBS :139MG%, 
HB: 13 GM%, 
TLC :11500, 
PLT: 2.08 LAKH

ON PRESENTATION TO OUR HOSPITAL

PT WAS UNCONSCIOUS

GCS :3/15 E1V1M1

BP :190/100 MMHG

PR :72 BPM

CVS :S1S2 +

MOTOR POWER :CANT BE ELICITED, TONE INCREASED IN ALL LIMBS

RELEXES:
R/L Biceps+ triceps+
Supinator_ knee3+ ankle3+
Cerebellar signs : can't be elicited.

PROVISINAL DIAGNOSIS:ACUTE INFRACTION IN CEREBELLAR HEMISPHERE WITH HAEMORRAGHIC TRANSFORMATION WITH HTN. 

F/U
Patient was taken to Amma hospital on 22/9/21 .He was unconscious and passed away on the same day.
